Ontario is investing $2.1B to connect 2 million people to primary care by 2029, with Community Health Centres (CHCs) positioned as a key delivery mechanism. This creates both unprecedented opportunity and pressure for organizations to scale operations, modernize systems, and innovate.

This session brings together CHC leaders, clinicians, and system stakeholders for a candid, practical conversation on the realities of delivering community-based care in this moment of transformation. Moving beyond high-level discussion, we’ll explore the structural challenges facing Ontario’s community health sector and identify actionable paths forward.

Drawing on insights from leaders across the province, the discussion will examine how organizations can modernize within constrained budgets, navigate complex funding environments, and build sustainable innovation capacity, without disrupting frontline care.
