About Event

Join a panel discussion hosted by Guaranteed’s Head of Medical and Clinical Affairs Dr. Raihan Faroqui as we bring together founder, investor, and clinician experts from Wellthy, Koda Health, Empassion Health, Town Hall Ventures, and Generator Ventures to learn how we can collectively empower individuals with serious illnesses to shape their care journey. We will focus on the future of tech-enabled, risk-based, and health equity forward advanced illness care planning and family caregiver support.

Beneficiaries with advanced & serious illness comprise 4% of the Medicare population, but account for 25% of the cost. 43M+ caregivers in the U.S. have provided unpaid care in the last 12 months and struggle to balance work obligations, their own health issues, and caregiving responsibilities. Advance care planning (ACP) which involves discussion, decisions and documentation about how an individual contemplates their death is completed by less than 30% of Americans, leading to medical interventions that may not align with individuals' values and desires.

Due to unmet needs and unresolved symptoms, people living with a serious illness are heavy utilizers of the health care system including: 911 calls, emergency department visits, hospitalizations and re-admissions, skilled nursing facility admissions, and expensive specialist procedures and interventions.

As healthcare stakeholders drive towards reducing costs, increasing quality, participating in risk-based arrangements, and focusing on underserved communities, empowering high needs and high cost seriously-ill patients and their family caregivers becomes a top priority.
