​PLAY WITH PURPOSE — YOUTH ATHLETE WAIVER

​Required for all youth athletes participating in Play With Purpose on September 19, 2026. If your child has been registered as part of a Youth 4v4 team, a parent or legal guardian must complete this form for each participating athlete before event day. Completing this waiver does not register or purchase a team entry. It provides the required individual athlete information, parent/guardian consent, liability acknowledgment, emergency information, and media authorization. NO COMPLETED WAIVER = NO COURT PARTICIPATION.
