Dros y 3.5 mlynedd diwethaf, rydym wedi bod yn gweithio'n agos gyda 11 o bobl ifanc sydd â phrofiad byw o ledled Gwent i gydgynllunio a siapio dulliau newydd i gefnogi iechyd meddwl a gwytnwch pobl ifanc, gan ddefnyddio methodoleg cynllunio gwasanaeth.

Un o ganlyniadau allweddol y gwaith yma oedd creu'r Gymuned Ymarfer gyntaf ar gyfer gweithwyr proffesiynol Gwent sydd yn cefnogi iechyd meddwl pobl ifanc.

Rydym yn cyfarfod wyneb yn wyneb ddwywaith y flwyddyn yng Nghasnewydd ac mae'r gymuned yn agored i unrhyw un sydd yn gweithio gyda phobl ifanc yng Ngwent - p'un a yw'ch rôl yn canolbwyntio'n benodol ar iechyd meddwl neu beidio.

Gyda'n gilydd, byddwn yn archwilio heriau a chyfleoedd i gefnogi iechyd meddwl pobl ifanc, creu gofod i rannu mewnwelediadau, myfyrio, dysgu gan ein gilydd a sbarduno syniadau newydd ar gyfer cydweithio.

Dyma ellir ei ddisgwyl:

  • Charlotte Thomas a Chris O'Connor (Perfformiad a Gwella GIG Cymru) - Diweddaru ar gynlluniau uchelgeisiol Cymru i drawsnewid cymorth iechyd meddwl a lles wrth gyflwyno Cymorth Iechyd Meddwl Mynediad Agored/Model Gofal Mewn Camau 2.0.

  • Diweddariadau prosiect Meddwl Ymlaen Gwent

  • Cyfleoedd rhwydweithio

  • Te, coffi a byrbrydau yn ystod y prynhawn

Diddordeb cyflwyno?

Eisiau hyrwyddo eich prosiect neu wasanaeth?

Os hoffech hyrwyddo eich prosiect neu wasanaeth, mae croeso i chi ddod â thaflenni neu nwyddau i'w rhannu gyda mynychwyr eraill.

Pwy ddylai fynychu?

Mae'r digwyddiad yma ar gyfer unrhyw un yng Ngwent sy'n rhan o lunio gwasanaethau i bobl ifanc - bod hynny trwy strategaeth, comisiynu, iechyd meddwl, lles, neu gyfranogiad ieuenctid. Croesawir uwch ymarferwyr, gwneuthurwyr penderfyniadau, a phawb sy'n gweithio gyda phobl ifanc. Mae yna groeso cynnes i bawb bob amser.

👉 We’re delighted to invite you to our next Mind Our Future Gwent Community of Practice event on Monday 12 October2026.

Over the past 4 years, we’ve been working closely with 11 young people with lived experience from across Gwent to co‑design and shape new approaches to supporting young people’s mental health and resilience, using a service‑design methodology.

One of the key outcomes of this work has been the creation of Gwent’s first Community of Practice for professionals supporting young people’s mental health.

We meet twice a year in person in Newport, and the Community is open to anyone working with young people in Gwent - whether or not your role focuses specifically on mental health.

Together, we will explore the challenges and opportunities in supporting young people’s mental health, creating space to share insights, reflect, learn from one another, and spark new ideas for collaboration.

Here’s what you can expect:

  • Charlotte Thomas and Chris O'Connor (NHS Wales Performance & Improvement) will be updating us on Wales’ ambitious plans to transform mental health and wellbeing support through the roll-out of the Open Access Mental Health Support/Stepped Care 2.0 Model.

  • More talks TBC

  • Networking opportunities

  • Tea, coffee and nibbles throughout the afternoon

Interested in presenting?

Our next CoP event will be held in spring 2027 and we’re looking for more speakers to share examples of good practice, insights, or challenges. If you’re interested in delivering a 20‑minute presentation, please contact [email protected].

Want to promote your project or service?

If you’d like to promote your project or service, you’re welcome to bring flyers or merch to share with other attendees.

Who should attend?

This event is for anyone in Gwent who plays a role in shaping services for young people - whether through strategy, commissioning, mental health, wellbeing, or youth participation. We welcome senior practitioners, decision-makers, and all professionals working with young people. Even if it’s your first time attending, you’ll be warmly welcomed.
