SGH SingHealth Community Hospitals (SCH)
Meet with Dr. Teo, Chiang Wen, Associate Consultant at SCH
This learning journey focuses on understanding how MDM currently operate at SCH and the operational load behind them.
Today, MDM meetings at SCH require a human scribe (often junior doctors) to manually document discussions for ~20 patients within an hour. While ambient scribe tools work well for 1:1 clinical encounters and meetings, they don’t yet fit the fast-paced, multi-patient MDM format.
Key challenges:
- High case volume + short turnaround — switching between cases requires stopping recording, running AI, saving output, and starting a new session each time.
- Manual correction + EMR transcription still needed, creating double-work.
- Limited workflow integration — not designed for rapid, structured patient-by-patient documentation.
Why this matters: A streamlined MDM documentation solution would eliminate the need for human scribes, reduce operational burden, and improve care coordination. SCH CMIO believes solving this could scale across geriatric and rehab MDMs in acute hospitals, making it a high-impact opportunity.
