An AI coding agent deleted a company's entire production database in nine seconds, then confessed it had violated every principle it was given. New York City shipped a chatbot that told small business owners they could take a cut of their workers' tips. A trading firm lost 440 million dollars in 45 minutes because the warning emails went out and nobody was on the other end of them.
Not one of those was a model failure. Every one was a governance gap with a name.
