About Event

Supporting proactive rehabilitation and frailty management within Integrated Neighbourhood Teams: lessons from digital clinical workflow implementation

This session will explore how digital clinical workflows can support rehabilitation teams to proactively identify, assess and manage people living with frailty and at risk of falls within Integrated Neighbourhood Teams.

We will share Safe Steps' approach to supporting Comprehensive Geriatric Assessment (CGA) through digital workflows that complement existing population health and risk stratification tools. The session will discuss how standardised assessments completed in under five minutes, automated personalised care plans, and integration with EMIS Web and SystmOne can reduce administrative burden, support multidisciplinary working and help protect community therapy and rehabilitation capacity.

We will also present learning from a recent pilot in Bury, which demonstrated a 38% reduction in falls and a 57% reduction in ambulance callouts, alongside our work to scale proactive frailty and falls management across St Helens, a National Neighbourhood Health Pioneer site.

Safe Steps was the overall winner of Health Innovation East Midlands' Pitch Your Product: MedTech 2026 Award, selected from eight of the region's most promising MedTech innovators. During the webinar, we will share the implementation approach and evidence that underpinned our winning submission, alongside practical lessons on implementing digital innovation within rehabilitation services, streamlining referrals, supporting multidisciplinary care and embedding new approaches into routine clinical practice with minimal additional training.
