Earlier this summer, HTN caught wind of a situation in Hawai'i that captures some of the most important healthcare themes of this moment: vertical integration and value-based payment.

The basic gist: HMSA, the dominant player in Hawai'i's insurance market, proposed backing off capitated payments to primary care providers, a move that drew enough provider pushback that the governor stepped in and delayed it to January 2027. At the same time, HMSA is pursuing a merger with Hawai'i Pacific Health, one of the larger health systems in the state, which would create a new entity, One Health Hawaii.

Jason Chang, president & CEO of The Queen's Health Systems, the largest private non-profit hospital in Honolulu, has been a prominent critic of the merger, arguing it would steer commercial patients toward the integrated system and leave other providers caring for the neediest patients at the lowest reimbursement rates. HTN will sit down with Jason to talk through the Hawai'i situation and what it signals about vertical integration and value-based care more broadly.
