🎯The Challenge

Chronic conditions (obesity, type 2 diabetes, cardiovascular disease) affect millions of people, yet most healthcare systems still treat them reactively and in-clinic. eMed has spent years proving that the opposite is possible: that patients managed at home, with the right tools, can achieve outcomes far beyond what traditional care delivers. We see 90% adherence rates (i.e. keeping to the recommended programme of medication and/or behaviour) where the industry averages 40–60%.

The question we're putting to you: what does the next generation of at-home chronic condition management look like, powered by AI?

We're not looking for incremental improvements to existing apps. We want genuinely novel ideas: new ways to support patients through long programmes, surfaces for monitoring and coaching that feel human, tools that make adherence effortless, or entirely new ways to think about how AI can sit alongside someone managing a long-term health condition.

🔨What to Build

Your submission should be an AI-powered tool or product concept for at-home (or non-clinic-based) chronic condition management. Challenges include (but are not limited to):

  • Inadequate at-home biomarker monitoring tools lead to immature data interpretation and missed critical signals.

  • How can we create AI that offers genuine, ongoing support for long-term health?

  • How do we provide better support for patients managing complex needs like heart or women's health?

  • How can we connect patients with their clinical teams without needing constant, real-time appointments?

  • How can we turn complex health data into simple, actionable advice for patients?

❓Judging

Submissions will be judged on:

  • User impact - Does this genuinely improve life for a patient managing a chronic condition? Is the need real and the solution meaningfully better than what exists?

  • Innovation - Is this a novel approach? We're looking for ideas that couldn't have existed two years ago, or that reframe the problem in a surprising way.

  • Feasibility - Could this be built and deployed in the real world?

  • Demo quality - Can you show it working, even in prototype form? A live demo or convincing walkthrough matters more than slides.
