“AI in Action: Practical Applications for Safety Net Providers” is a monthly Zoom series, co-hosted by Center for Care Innovations the Health AI Partnership, and AllianceChicago, dedicated to exploring research and implementation of AI based solutions that address the unique challenges and opportunities within Federally Qualified Health Centers and safety net healthcare organizations, promoting equitable access and improved patient outcomes.

As healthcare continues to evolve in the age of data and innovation, the South Carolina Health Center Controlled Network (SCHCCN) is leading the way with the launch of its AI-Powered Quality Risk Model—a transformative tool designed to help Community Health Centers turn information into action. This new model harnesses the power of artificial intelligence to identify patients at higher risk for adverse health outcomes, uncover care gaps, and support proactive, data-driven interventions. By integrating predictive analytics directly into quality improvement workflows, SCHCCN is empowering their participating health centers to move from reactive care to precision population health management, advancing equity, outcomes, and value across South Carolina’s primary care network.

Speakers

Chandra Beasley, MBA, MHRM, MNSA, MPA, CLSSGB

Chandra Beasley is the Director of Information Technology at the South Carolina Primary Health Care Association (SCPHCA) and serves as the Program Director for the South Carolina Health Center Controlled Network (SCHCCN). Ms. Beasley has worked for the SCPHCA/SCHCCN for seven years and has more than 15 years of experience in the health information technology field. In prior roles, she gained extensive experience in EHR architecture build and design, HIE implementation/optimization, HL7 and reporting support, and quality assurance testing.

She is an adjunct professor at Benedict College and holds Master’s Degrees in Business Administration, Network Systems Administration, Public Administration, and Human Resource Management, all from the Keller Graduate School of Management. Ms. Beasley is also a Certified Lean Six Sigma Green Belt and is currently pursuing a Doctoral degree in Health Administration from Capella University.
