Automating Post Discharge Home Support - Peer Learning Session

​Join us for an in-person peer learning session hosted by CareForMe and Age UK Lambeth where we will share practical lessons from running an automated at‑home discharge support service for pathway 0 & 1 patients across Lambeth.

​We’ll cover what worked, what didn’t and the operational steps taken to set up, automate and scale the service, plus lived experience from volunteers who supported patients at home.

​The session will include a short presentation from members from Lambeth council, Age UK Lambeth and CareForMe, volunteer reflections, and informal roundtable discussions to dig into implementation challenges and solutions.

​Afternoon tea and refreshments will be provided. Come to 10 York Road, London, ready to ask questions, compare approaches and take away concrete ideas you can apply locally.
